Here are the items you will need to complete an application for Social Security disability benefits.

• Your Social Security number.
• Names, addresses and phone numbers of your doctors, caseworkers, hospitals and clinics for the last for the last year. If you have had surgeries or other important treatment before then, you may need and address of those medical providers as well. 
• Names and dosage of all the medicine you take.
• Your work history for the past 15 years including the employer's name, the type of work you performed and your pay rate.
• A copy of your most recent W-2 Form (Wage and Tax Statement) or, if you are self-employed, your federal tax return for the past year.
• US naturalization papers (if applicable).
• US military discharge papers (if applicable).
• Check or savings account numbers and bank routing numbers for direct deposit (a check or deposit slip will have this information).
• The date married for current and all prior marriages.
• Spouse’s social security number (for current & prior marriages).
• Spouse’s date of birth (for current & prior marriages).
• Spouse’s maiden name (for current & prior marriages).
• City and state where married (for current & prior marriages).
• Date marriage ended.
• City and state married ended (for current & prior marriages).
• Name, date of birth, and current addresses for all children under age 22.

Social Security's website still lists that you should have your "birth or baptismal certificate." However, in our experience with online Social Security application, you do not need this to being your application.
