First, always make sure you are following the correct process. Any denial or decision which you are appealing should include a statement outlining your appeal rights including what needs to be filed (whether by form or online), where the document needs to be filed, as well as the time limits.
The easiest way to request a hearing is file online at https://www.socialsecurity.gov/disability/appeal. Note: even if you complete an appeal online, you still need to submit the following by mail to Social Security:
- A cover sheet provided toward the end of the online request for hearing process. You will be instructed to print the cover sheet and mail it along with any other documents to a Social Security office listed on the cover sheet.
- A signed "Authorization to Release Information to Social Security" form SSA-827.
- If you are helping someone as a representative on their Social Security case and you want Social Security to be able to release information to you, you will also need to provide a SSA-1696 Appointment of Representative form.
If you are unable to complete an online appeal, you can use Social Security's paper forms. Note: check with your Social Security office to confirm that they will accept a paper appeal.
- Request for Hearing, form HA-501.
- Disability Report-Appeal, form SSA 3341.
- Authorization to Release Information to Social Security, form SSA-827.
- Optional: SSA-1696 Appointment of Representative form if appointing a representative.
